Clinical Efficacy of Single Measurement of Serum Progesterone Level for the Early Diagnosis of Ectopic Pregnancy

Abstract

|
In order to evaluate the usefulness of single measurement of serum progesterone concentration in the differentiation of normal intrauterine and ectopic pregnancies, 106 patients(30 of pathologically proven ectopic pregnancies and 76 of normal
intrauterine pregnancies) were analyzed. Serum progesterone levels were measured during 4-8 gestational weeks in normal intrauterine and in ectopic pregnancies preoperatively.
The serum progesterone levels did not change significantly during early normal pregnancies. The serum levels(ng/ml, Mean¡¾SEM) of progesterone with ectopic pregnancies (7.54¡¾1.05) was significantly lower than that measured in intrauterine
pregnancies(21.81¡¾0.72, p<0.05). Especially in early pregnancies with serum ¥â-hCG<1,000 mIU/ml when gestational sac is usually invisible by ultrasound examination, serum progesterone level with cut off value of 15 ng/ml can differentiate
intrauterine
from ectopic pregnancies with a very high sensitivity(100%). In conclusion, single measurement of serum progesterone level in early pregnancy deserves a additional diagnostic labaratory tool for the earlier detection of ectopic pregnancy.
